The most recent inspection on May 27, 2025, found the facility in compliance with all regulations after reviewing corrections from the prior April 17, 2025, survey, which had cited deficiencies. Earlier inspections showed a pattern of issues related to resident care, professional standards, food safety, infection control, and life safety code compliance, including a substantiated complaint investigation in July 2023 that identified failures in dignity and respect, medication management, and infection prevention. The facility had an immediate jeopardy finding related to food safety and resident care in May 2024 but corrected those deficiencies upon revisit. Complaint investigations were mostly unsubstantiated except for the July 2023 case. Recent inspections indicate improvement as the facility has addressed prior deficiencies and returned to compliance.
