This facility’s inspection record is problematic, with repeated failures in safe resident transfers and lack of a licensed Administrator contributing to serious injuries and regulatory violations. On December 23, 2025, inspectors substantiated a complaint after a resident suffered a femur fracture due to improper transfer without required equipment or assistance, and the facility lacked a licensed Administrator throughout the survey.
