Epworth Villa Health Services is a 130-bed continuum of care facility at 14901 North Penn Avenue, Oklahoma City, Oklahoma County, Oklahoma, operated by Central Oklahoma United Methodist Retirement Facility Inc, a nonprofit organization. The community offers independent living, assisted living, memory care with 40 dedicated Alzheimer’s and dementia beds, skilled nursing, long-term care, and respite care with 24-hour staffing, rehabilitation services, and a partnership with OU Health. Current occupancy is 108 of 130 beds (83 percent) as of June 2025, with growth from 60 residents in May 2019 to peaks of 112 in June 2022 and 2023. The facility accepts Medicare but not Medicaid.
Nursing care averages 34 percent above the Oklahoma state benchmark. CMS ratings stand at 4/5 for Health Inspection, 4/5 for Staffing, and 4/5 for Quality Measures.
Medication management practices have been the primary focus of recent deficiencies; fourteen inspections from May 2019 through November 2025 yield 2.7 deficiencies per year, 45 percent better than the Oklahoma average of 4.9. Recent complaint investigations show mixed results: the June 2025 elopement allegation was unsubstantiated; the February 2025 investigation found 3 deficiencies related to medication cart security and nebulizer storage; and the January 2024 abuse allegation was unsubstantiated. Significant deficiencies clustered in 2023 and 2022. The November 2023 complaint investigation documented 6 deficiencies spanning bankruptcy reporting, medication management including discontinued narcotic reconciliation and unauthorized administration, and comprehensive assessment failures following falls.
The June 2023 relicensure survey identified 4 deficiencies: incomplete resident interviews in comprehensive assessments, unsafe shower water temperature at 143 degrees Fahrenheit, missing CPR training for direct care staff, and inadequate abuse training timelines. A July 2023 follow-up confirmed all June deficiencies were corrected. The December 2022 complaint investigation substantiated 2 deficiencies in medication reorder and reconciliation program and physician order compliance. The June 2022 complaint investigation substantiated 3 deficiencies including medication administration errors and neglect resulting in resident injury; the May 2022 complaint investigation found all allegations unsubstantiated.
Three consecutive renewal licenses (November 2022, August 2021, August 2020, September 2019) found zero deficiencies. The May 2019 relicensure survey identified 1 deficiency regarding medication administration record documentation. No fines or immediate jeopardy findings appear in the available record.
Occupancy has grown steadily from 60 to 108 residents, suggesting sustained market demand and operational confidence. Oklahoma City is the state capital; Integris Baptist Medical Center and OU Medicine are major nearby acute-care resources.
Families should ask administrators directly about medication administration protocols, staff training requirements, and corrective measures taken following the February 2025 medication cart security findings.
