The community offers independent living, assisted living, memory care with 40 dedicated Alzheimer’s and dementia beds, skilled nursing, long-term care, and respite care with 24-hour staffing, rehabilitation services, and a partnership with OU Health. A July 2023 follow-up confirmed all June deficiencies were corrected. The December 2022 complaint investigation substantiated 2 deficiencies in medication reorder and reconciliation program and physician order compliance. The June 2022 complaint investigation substantiated 3 deficiencies including medication administration errors and neglect resulting in resident injury; the May 2022 complaint investigation found all allegations unsubstantiated.

Three consecutive renewal licenses (November 2022, August 2021, August 2020, September 2019) found zero deficiencies. The May 2019 relicensure survey identified 1 deficiency regarding medication administration record documentation. No fines or immediate jeopardy findings appear in the available record.
