The most recent inspection on March 16, 2023, identified multiple deficiencies related to staff training, administration oversight, resident protection from abuse and neglect, medication administration, use of physical restraints, and timely reporting to regulatory authorities. Earlier inspections showed ongoing issues primarily with building and fire safety code compliance, including fire alarm placement, sprinkler system maintenance, fire-rated walls and ceilings, and exit path obstructions.
