Inspection Report Summary for Holmes Lake Rehabilitation & Care Center

The most recent inspection on March 31, 2025, confirmed that Holmes Lake Rehabilitation & Care Center met statutory requirements for licensure renewal without any deficiencies. Earlier inspections showed a mixed record with several citations related to resident care, fall prevention, infection control, and life safety code compliance. Notable issues included failure to assess and implement fall prevention interventions, incomplete fire safety measures, and documentation problems, with one license probation period in 2019 due to fall-related deficiencies. Complaint investigations were mostly unsubstantiated, except for a substantiated failure to prevent falls leading to injury in 2019 and a substantiated delay in pain medication delivery in 2011; enforcement actions included a 90-day probation in 2019 but no fines or license revocations were listed. The facility’s recent clean renewal inspections suggest improvement following earlier compliance challenges.
