Case management services issues appeared in June 2022. Since August 2024 renewal, the facility has demonstrated sustained compliance with zero deficiencies across 5 consecutive complaint investigations. Verify directly with the New York Department of Health that all corrective actions from the August 2024 renewal inspection have been fully implemented and documented
