One unrelated deficiency was cited: failure to ensure two of three dietary staff responsible for food preparation had attended an approved food service training program. Substantial compliance was confirmed by July 10, 2023. A December 15, 2023, initial licensing inspection found no deficiencies.
A July 10, 2024, license renewal found no deficiencies. A November 20, 2025, relicensure survey with complaint investigations addressed allegations of inadequate staffing, failure to involve legal guardians in care planning, inadequate supervision, failure to provide discharge notices and paperwork, failure to timely assess and intervene for resident condition changes, and failure to maintain a safe, clean environment free of pests; all complaints were unsubstantiated and no deficiencies were cited.
