Prior inspections showed a pattern of deficiencies related mainly to resident dignity and privacy, medication management, infection control, and environmental safety, particularly noted in the April 30, 2025 survey. Complaint investigations in 2023 and 2019 included substantiated findings involving failure to provide adequate supervision leading to resident abuse and failure to report alleged mistreatment, respectively.
