The facility’s inspection record is mixed, showing repeated deficiencies in resident privacy, abuse prevention, feeding tube care, and behavioral health services, with substantiated verbal abuse and delayed abuse reporting cited on July 24, 2025. That inspection found 10 deficiencies including failure to close electronic medication records, verbal abuse by staff, late physical abuse reports, incomplete care plans, and unsafe food storage.
