This facility’s inspection record is problematic due to repeated failures to protect residents from abuse, inadequate infection control, and poor staff training, culminating in substantiated sexual abuse in its most recent complaint investigation on March 6, 2025. Inspectors cited seven deficiencies in that investigation, including failure to protect Resident #16 from sexual abuse by Resident #58, inadequate discharge procedures, medication storage issues, and poor infection control practices. Earlier inspections in 2023 also flagged multiple abuse incidents, deficient dementia and respiratory care, and persistent infection control lapses.
