Staffing Info Measures average nursing staff hours per resident per day, including Registered Nurses (RNs) and total nursing staff. Ratings are adjusted based on the level of care residents require and are compared to state and national benchmarks.
Quality Measures Info Based on clinical and physical health indicators reported to CMS, such as hospital readmissions, falls, pressure ulcers, and improvements in mobility. These measures reflect how well residents' health needs are being managed.

Staff typeHours / Day / Residentvs state avg
Registered Nurse (RN) Info RNs hold the highest nursing license and can assess residents, interpret test results, and direct care plans. More RN hours per day often signals stronger clinical oversight and faster response to health changes. 20m per day ▼ 52% State avg: 41m per day · National avg: 41m per day
LPN / LVN Info Licensed Practical Nurses (LPNs) or Licensed Vocational Nurses (LVNs) deliver routine hands-on care — medication administration, wound dressing, and monitoring vital signs. They work under RN supervision and make up a large share of daily bedside care. 1h 2m per day ▲ 11% State avg: 56m per day · National avg: 52m per day
Nurse Aide Info Certified Nurse Aides (CNAs) provide the most direct day-to-day assistance: bathing, dressing, feeding, and mobility. Nurse aide hours are typically the largest staffing category and directly affect residents' quality of life. 2h 15m per day ■ Avg State avg: 2h 14m per day · National avg: 2h 21m per day
Weekend Total Nursing Info Combined nursing hours (RN + LPN + Nurse Aide) per resident per day on weekends. Staffing often drops on weekends — this figure reveals whether the facility maintains adequate coverage outside of weekday hours. 3h 14m per day ▼ 6% State avg: 3h 27m per day · National avg: 3h 26m per day
Physical Therapist Info Hours per resident per day provided by licensed Physical Therapists (PTs) or PT Assistants. PT services help residents recover mobility after injury or illness and are especially important for post-acute (short-stay) rehabilitation. 5m per day ▼ 24% State avg: 6m per day · National avg: 4m per day

I have been at preferred care for my rehab. I’m starting my third week. When I arrived. I was impressed with the cleanliness of the rooms the floors that there was no odor or whatsoever my room was mopped and disinfected at least once a day. The staff was very pleasant knowledgeable so all in all I would say that I would come back here again if I needed their help, the rehab department was very helpful and knowledgeable at their trade. This nursing home is no different than any other when it comes to their culinary department. I am well aware that most of the people there are on restricted diets or don’t eat with spices cause it bothers her stomach I found that having my own salt and pepper shaker and Italian spices I brought from home Really help things out. I’d like to say that the staff has been very accommodating when I have a doctors appointment are needed to go for some testing. They made sure that I got to go where I needed to be for doctors visits and testing in many cases, they brought the x-rays machines right into my room. So I will close with saying that I would recommend this facility to all my friends and acquaintances. What do you like most about living here?
