Registered Nurse (RN) Info RNs hold the highest nursing license and can assess residents, interpret test results, and direct care plans. More RN hours per day often signals stronger clinical oversight and faster response to health changes. 22m per day ▼ 15% State avg: 26m per day · National avg: 41m per day
LPN / LVN Info Licensed Practical Nurses (LPNs) or Licensed Vocational Nurses (LVNs) deliver routine hands-on care — medication administration, wound dressing, and monitoring vital signs. They work under RN supervision and make up a large share of daily bedside care. 44m per day ▼ 23% State avg: 57m per day · National avg: 52m per day
Nurse Aide Info Certified Nurse Aides (CNAs) provide the most direct day-to-day assistance: bathing, dressing, feeding, and mobility. Nurse aide hours are typically the largest staffing category and directly affect residents' quality of life. 1h 53m per day ▼ 7% State avg: 2h 1m per day · National avg: 2h 21m per day
Total Nursing (Weekend) Info Combined nursing hours (RN + LPN + Nurse Aide) per resident per day on weekends. Inspection Reports Summary Info An editor-reviewed summary of the themes and findings across this facility's recent inspection reports.

The most recent inspection on November 20, 2025, identified a deficiency related to the facility’s failure to keep a resident’s call light within reach. Earlier inspections showed a pattern of deficiencies involving resident care issues such as inadequate supervision leading to injury, incomplete care plans, and failures in wound care and abuse reporting procedures. Complaint investigations substantiated concerns about care plan adherence, injury prevention, and abuse notification, with Immediate Jeopardy findings noted in two prior inspections but later removed. Enforcement actions such as suspensions and terminations of staff were documented, but fines or license suspensions were not listed in the available reports. The facility’s inspection history reflects ongoing challenges with care planning and resident safety, with some corrective actions taken following serious findings.

Dolores Schriver
When my family and I found out that we needed to take my husband to a hospice facility, it was like a bad dream. It’s hard to get your mind straight when realizing that you need to try and find the best place for your loved one in this situation. We were fortunate enough to choose the River Ridge Facility. Scared and confused is how I would describe the state that I was in when first arriving. I knew nothing about this type of facility. My husband was put in a private room that had just been remodeled. It was quiet and large enough for our family and friends to gather. The nurses and the aids were punctual with any of his medication regiments. The aids were very vigilant about checking on him to make sure he was doing all right, or just to visit. The administration, nursing staff, housekeeping, laundry, and kitchen employees were always very friendly and did whatever they could to make things easier for our family. We are blessed to have made new friends at River Ridge. If you have to take a loved one to a nursing home or hospice facility I cannot recommend River Ridge enough.
