Registered Nurse (RN) Info RNs hold the highest nursing license and can assess residents, interpret test results, and direct care plans. Inspection Reports Summary Info An editor-reviewed summary of the themes and findings across this facility's recent inspection reports.

Riverside Health Care Center’s inspection record is mixed, with repeated deficiencies in resident supervision, abuse prevention, and care planning. The most recent inspection on December 30, 2025, substantiated physical abuse causing injury and cited failure to protect a resident, while earlier complaints documented elopement risk reassessment failures and inadequate mental health care. Although some prior violations were corrected, ongoing issues with abuse investigations, medication errors, and neglect indicate inconsistent compliance and safety concerns over time.
