The regulatory record reflects generally good compliance with deficiency clusters in food service, staff training, and assessment practices. Over five years, Saint Simeons logged seven deficiencies, averaging 1.4 annually, 71 percent better than Oklahoma’s 4.9-deficiency benchmark. An April 11, 2019, relicensure survey cited two deficiencies: failure to ensure three of nine sampled kitchen staff had documented evidence of food service training prior to food preparation, and failure to initiate fingerprint-based national background checks for three of nine sampled employees upon hire. Substantial compliance was achieved by May 17, 2019.

A June 8, 2021, complaint investigation addressing allegations of failure to ensure residents’ right to visitors and failure to follow COVID-19 infection control practices found no deficiencies; both allegations were unsubstantiated. A March 13, 2023, complaint investigation addressing failure to have a properly functioning call system found the allegation unsubstantiated with no deficiencies. A June 6, 2023, relicensure survey cited five deficiencies: failure to ensure food was stored, prepared, and served in a sanitary manner with uncovered trash cans and lack of beard guards for staff with facial hair, failure to ensure potentially hazardous leftovers were disposed of within 24 hours, failure to ensure resident assessments were coordinated and signed by a registered nurse or physician for two of eight sampled residents, failure to ensure comprehensive assessments included personal interviews for two of eight sampled residents, and failure to ensure direct care staff were trained in first aid and cardiopulmonary resuscitation for four of five recently hired staff members. A December 1, 2023, complaint investigation addressing abuse allegations found no deficiencies.

An October 10, 2024, complaint investigation addressing physical abuse allegations found no deficiencies. A January 3, 2025, complaint investigation addressing abuse and property misappropriation allegations found no deficiencies. The facility’s occupancy has remained between 47 and 73 residents in recent inspections. No fines or license suspensions are recorded.

The facility provides assisted living, long-term care, memory care, Parkinson’s care, respite care, and rehabilitation services. Licensed capacity is 88 beds with current occupancy of 73 residents (83 percent). Nursing hours 25 percent above Oklahoma benchmark. Administrator Angela Green.

Specialized programs include Saint Simeon’s Catered Living, Medical Resort, and Post-Rehab Bridge to Home.

The June 2023 deficiency cluster spanning food sanitation, staff training on first aid and CPR, and resident assessment completion required attention to operational practices. Subsequent complaint investigations from December 2023 through January 2025 found no deficiencies, signaling improved compliance

Before placement, families should verify corrective measures and current protocols for food service sanitation, staff training documentation, and assessment completion following the June 2023 findings.
