A January 26, 2023, complaint investigation substantiated three deficiencies related to failure to follow CMS COVID exposure guidelines, failure to perform monthly medication reviews for three residents, and failure to allow private visitation during COVID lockdown for three residents. Substantial compliance was achieved upon revisit.

A September 21, 2023, complaint investigation identified one deficiency for failure to document medication administration for three of nine sampled residents, with missing documentation for multiple evening doses. The facility reported internet outages causing loss of medication administration records with no alternative documentation methods used. Correction was achieved by December 6, 2023, with a revisit on December 7, 2023.

A September 25, 2024, complaint investigation addressing abuse and staffing allegations found no deficiencies. The facility’s occupancy has grown from 42 residents in July 2019 to 62 residents in September 2024. No fines or license suspensions are recorded.

The facility provides assisted living and memory care with 24-hour staffing. Licensed capacity is 100 beds with current occupancy of 62 residents (62 percent). Occupancy has shown steady growth. Nursing hours 28 percent above Oklahoma benchmark.
