Preserve at Woodland Hills, a 740‑capacity group home in Woodland Hills, California, has been the subject of two substantiated complaints investigated by the California Department of Social Services, Community Care Licensing Division. The first investigation, conducted in March 2022, found that staff supervision failures resulted in at least two elopement incidents involving Resident #1 (R1). The first elopement on 12/31/2021 involved an inoperable delayed‑egress door at the rear of the facility, while the second on 2/23/2022 occurred through the front entrance, which lacks a delayed‑egress system. In both cases, staff failed to detect the resident’s exit promptly, and notifications from the door‑open alarm were ignored. Medication audit findings revealed multiple missed dosages—nine missed Eliquis doses, more than a dozen missed vitamins, and eleven missed senna doses—due to the medication not being available on site. The facility also lacked adequate documentation for PRN medications and had staff missing current first‑aid/CPR certification, job applications, and health screenings. Deficiencies cited under CCR 87465(a)(4) and CCR 87412(a) require the administration of in‑service training on medication protocols and a comprehensive personnel file audit, both due within 10–14 days of the inspection.

Across both inspections, a pattern emerges of inadequate staff training, insufficient monitoring of resident safety, and lapses in record‑keeping.
