Preserve at Woodland Hills, a 740‑capacity group home in Woodland Hills, California, has been the subject of two substantiated complaints investigated by the California Department of Social Services, Community Care Licensing Division. The first investigation, conducted in March 2022, found that staff supervision failures resulted in at least two elopement incidents involving Resident #1 (R1). The first elopement on 12/31/2021 involved an inoperable delayed‑egress door at the rear of the facility, while the second on 2/23/2022 occurred through the front entrance, which lacks a delayed‑egress system. In both cases, staff failed to detect the resident’s exit promptly, and notifications from the door‑open alarm were ignored. Deficiencies cited under CCR 87465(a)(4) and CCR 87412(a) require the administration of in‑service training on medication protocols and a comprehensive personnel file audit, both due within 10–14 days of the inspection.
