An investigation on December 16, 2022, substantiated that after Resident A experienced a stroke on the night of October 9 or early morning of October 10, 2022, multiple staff observed stroke-like symptoms but medical attention was delayed for several hours. The facility failed to complete and submit timely incident reports with required information.

An investigation on December 2, 2022, substantiated that Resident A had an unwitnessed fall with unexplained injuries. The facility failed to check alarms and assist with toileting every two hours as required. Hospital transfer was delayed, and the family contacted before a physician. Documentation shows the facility lacked an organized program ensuring resident protection.

An October 2023 administrative review found substantial regulatory compliance. No enforcement actions or fines are recorded. The license.

It is imperative to request clarification on current protocols for timely emergency response, resident alarm monitoring, incident reporting procedures and timeframes, medication administration oversight, and resident protection programs. Verify that all deficiencies cited in the May 2024 renewal inspection have been resolved and confirm oxygen storage safety procedures before making a placement decision.

Families should ask administrators directly about the specific corrective actions implemented following the two substantiated fall and stroke investigations in late 2022.
