WALNUT PLACE PC UNIT underwent a comprehensive inspection on July 12, 2022, resulting in six state standard violations. Of these, four were classified as Health Code deficiencies, including the failure to assess or develop service plans for residents and inadequate documentation of injuries or illnesses. The remaining two violations fell under the Life Safety Code, which also included three older citations from August 17, 2021, related to building maintenance and oxygen storage procedures. These latter issues had been corrected by September 10, 2021. No Health Code deficiencies were listed among the older citations.
