Inspection Report Summary for Vintage Faire Residential

On May 16, 2025, the California Department of Social Services Community Care Licensing Division conducted an unannounced pre‑licensing evaluation of VINTAGE FAIRE RESIDENTIAL (Facility No. 502701602), a 49‑bed assisted‑living facility in Modesto. Licensing Program Analyst Renee Campbell inspected the entire physical plant—including resident units, common areas, kitchen, dining and laundry spaces—as well as outdoor walkways and safety equipment. The visit was part of a change‑of‑ownership review for Deer Tail Living Inc. DBA Vintage Faire Residential. Campbell found no hazards or non‑compliance with Title 22 regulations; resident units were adequately furnished, private bathrooms were equipped with grab bars and non‑skid flooring, and all required detectors, sprinklers and medication storage systems were operational. The kitchen and dining areas met repair and food‑storage standards, and records for residents and staff were properly maintained. No deficiencies were cited, and the pre‑licensing evaluation was declared complete with the facility in substantial compliance.

Three months later, on August 25, 2025, a second unannounced visit of a different type was carried out by Licensing Program Analyst Jason Lund. This case‑management visit focused on staff conduct after a CDSS investigation. Lund delivered an Immediate Exclusion Order to the facility concerning staff member S1, who had engaged in conduct inimical to the role of caregiver. The order bars S1 from any contact with residents at any licensed facility. The census at the time of the visit was 43 residents, down from the facility’s 49‑bed capacity. An exit interview with Administrator Prithika Singh was conducted, and the exclusion order was formally documented in the report.

Together, the reports illustrate VINTAGE FAIRE RESIDENTIAL’s strong adherence to safety and operational standards, as evidenced by the unblemished pre‑licensing review. However, the August visit reveals an internal risk that required decisive regulatory action. The pattern underscores the importance of continuous staff monitoring and the capacity of the licensing agency to intervene swiftly when caregiver conduct threatens resident safety. The facility’s ability to resolve the exclusion promptly and maintain ongoing compliance will be key to its continued licensing status.
