Inspection Report Summary for Serra Sol Memory Care

The California Department of Social Services Community Care Licensing (CCLD) conducted a series of inspections at SERRA SOL, a residential care facility for the elderly with a capacity of 70 residents. Beginning with a telephone “COMP II” check in May 2021, the administrator, Janelle Lopez, confirmed understanding of licensing requirements and was directed to submit a signed LIC 809. The same month an unannounced pre‑licensing inspection verified that the two‑story building, its central courtyard, and all resident rooms were safe, furnished, and equipped with functional call systems, fire and carbon‑monoxide detectors, and adequate food and medication storage. The facility met Title 22 Division 6 standards and was cleared for licensure.

Following licensure, CCLD performed a post‑licensing visit in March 2022 that found no deficiencies. The inspector noted proper furnishings, functioning hot‑water systems, a recent fire drill, and a pending mitigation plan. Subsequent annual inspections in June 2022 and June 2024 similarly reported no safety or compliance issues; fire extinguishers were charged, hand‑sanitizing stations were present, and an infection‑control plan had been submitted. The facility’s administrative leadership transitioned to Lindsay Schroeder by the 2024 visit.

An unannounced case‑management inspection in August 2022 addressed an incident involving Resident 1 (R1) who became aggressive toward staff and another resident. The incident was resolved with a 911 call, physician‑adjusted medication, and no reported injuries. The inspector cited no deficiencies, and the facility promptly documented the event and notified required parties. Across all visits, SERRA SOL consistently demonstrated adherence to safety, staffing, and programmatic requirements, maintaining compliance and a safe environment for its residents.
