Inspection Report Summary for Two Palms Care Center

The California Department of Social Services Community Care Licensing Division conducted four unannounced investigations at Terraces at Park Marino (facility number 197602744). In each case a formal complaint was received, the evaluator interviewed residents, staff, and administrators, reviewed relevant records, and held exit interviews with the facility representative.

The first investigation, triggered by a complaint in September 2020, concerned allegations that residents’ personal items and money were stolen while in care. Interviews with nine residents and six staff members revealed that only two residents reported missing small items, none of which had been reported to management, and no incident or police reports were found in the facility’s files. The evaluator noted that the facility had assisted one resident in obtaining a power of attorney and re‑assessing her financial management, but the evidence did not substantiate the theft claim. The second investigation, received in August 2021, focused on staff failure to provide accurate medical records to a resident’s hospital and inadequate communication with the medical provider. Although a front‑desk receptionist had mistakenly faxed the wrong documents, the evaluator could not find evidence that this error led to a violation of regulations, and the staff’s training records and fax logs did not show a pattern of non‑compliance. The third investigation, filed in April 2021 and conducted in August 2022, addressed a claim of inadequate supervision that resulted in multiple falls and injury for a deceased resident. Staff described a system of shift‑change debriefs, a communication binder, and a medication‑room whiteboard used to track resident status. No additional falls were reported after the incidents in question, and the evaluator found no documentation indicating a lapse in supervision.

Across all three investigations, the evaluator reached the same conclusion: the allegations were unsubstantiated. The repeated pattern—complaints centered on resident belongings, medical record handling, and supervision—suggests persistent concerns about staff communication and procedural compliance, yet no preponderance of evidence was found to support regulatory violations. The facility’s responses—training logs, incident reporting, and efforts to seek medical attention—were noted, but the investigations highlighted gaps that warrant ongoing monitoring to ensure resident safety and staff accountability.
