The April 14, 2025 follow-up confirmed substantiated resident-to-resident abuse and failure to conduct overnight fire drills, resulting in two deficiencies related to safety and supervision.
December 6, 2023 renewal inspection cited 14 deficiencies involving staff qualifications, confidentiality breaches, safety hazards, and documentation errors, all corrected by January 2024.
