Inspectors found six deficiencies: multiple residents with missed medication doses but no documentation of reasons; staff failing to record when medications were given; three employees lacking timely TB testing; one caregiver without position-specific training records; undated opened food containers in freezer storage; and cleaning chemicals in an unlocked kitchen cabinet accessible to cognitively impaired residents.
The facility received zero deficiencies at its September 2023 renewal, suggesting rapid deterioration in compliance over the following year.

It serves residents aged 60 and above with assisted living and memory care services. Twenty-four-hour staffing includes licensed nurses and certified nursing assistants. Cortney Banker provides leadership, and the facility holds a BBB rating of A+.
The Reflections’ inspection record presents a mixed compliance picture that warrants attention. Over four years since 2020, the facility accumulated eight deficiencies at 2 per year, which is 62 percent better than Michigan’s state average of 5.2 annually.
