The apparent strength of this statistic, however, masks recent deterioration.

A May 2023 administrative review confirmed substantial compliance and renewal, yet only seven months later the June 2024 renewal inspection identified five deficiencies: medication administration planning for Resident A lacked detail regarding Lorazepam administration; the facility failed to conduct the annual tuberculosis risk assessment; employee competency checklists were incomplete for certain Resident Care Associates and Medication Technicians; items in the walk-in refrigerator lacked dates; and cleaning products remained unsecured in the dining room.

More significantly, a December 2022 complaint investigation substantiated serious violations of discharge procedures and medication administration: the facility discharged Resident D without providing written less than 24-hour notice and did not inform the resident or authorized representative of the right to file a complaint; staff failed to administer prescribed as-needed Risperdal despite documented agitation and aggression; the service plan contained no behavioral guidance or medication instructions.
