This is a continuing care retirement community on Franklin Street in Grand Rapids’ Eastown neighborhood, and it holds nursing home, assisted living, independent living, memory care, and skilled nursing under one roof across 107 beds in a 3-story building. Residents who enter at one level of need can remain as that need changes. The average stay is 424 days, and Corewell Health Blodgett Hospital is 0.43 miles away.
Thirty-six of those 107 beds are currently occupied. Staffing runs 24 hours a day. Total nursing time is 3 hours and 28 minutes per resident per day: registered nurses at 26 minutes, LPNs at 26 minutes, nurse aides at 2 hours and 4 minutes. The amenities here are genuinely atypical for a nursing-level community. Multiple dining venues serve chef-prepared meals. Outdoor space includes a rose garden, walking paths, and pet-friendly grounds.
An active resident council meets regularly. Rachel Romero is the Executive Director; Clark Retirement Community Inc. operates the facility. The facility accepts Medicare, Medicaid, and private pay. State inspections have historically focused on themes of care planning and documentation.

The facility opened in 2018 with substantial compliance. The February 2023 renewal found zero deficiencies. This record collapsed in July 2023. July 20, 2023, investigation documented restraint violations and systemic documentation failures: resident B was restrained in a wheelchair with a seatbelt without a physician order specifying reasoning or timeframe; resident C was restrained with gloves or mitts on hands without physician order; resident B’s admission agreement was invalid because it was signed by a relative without activated durable power of attorney rather than by the resident.
The facility failed to develop an appropriate service plan for Resident B due to the same lack of activated durable power of attorney. No tuberculosis screening was on record for Resident B within 12 months before admission. The facility refused to accept Residents B and C back after hospital discharge, constituting improper discharge practice.
March 2024 renewal found continued problems. Resident A’s tuberculosis screening was completed after admission, not before. Employee A lacked evidence of tuberculosis screening within 10 days of hire.
The July 2023 deficiencies involved use of restraints without clinical authorization, acceptance of legally invalid admission documents, and refusal of appropriate hospital discharge placements. Ask to see the most recent state inspection report, the move-out / level-of-care-change policy, and a sample monthly bill that lists every fee.
