Overview of Fountain View Retirement Village Grant

Fountain View Retirement Village Grant is a 38-bed nonprofit assisted living community at 50 S Maple Street, Grant, Newaygo County, MI, with 32 years of operating history. The facility ranks 305th of 437 Michigan communities by bed count and 190th of 702 for walkability with a score of 53. Sarah Maxim-Gerard is administrator and Connie Clauson is authorized representative.

The facility provides supportive care, memory care, and respite services with 24-hour staffing and rehabilitation services. Residents receive three nutritious meals daily with special diet options. Services include religious and spiritual care, social and recreational activities, housekeeping and laundry, assistance with activities of daily living, medication assistance, and care coordination with outside agencies. Administrative hours are Monday through Friday, 8:30 AM to 5:00 PM.

Inspection records from July 2010 through February 2023 documented consistent compliance. The February 11, 2023 renewal inspection found zero deficiencies with substantial compliance determination. However, the February 13, 2024 renewal inspection identified two equipment-related deficiencies: loose bed rails with entrapment gaps and unsecured bed cane, and oxygen tanks not properly secured in holsters with multiple tanks freestanding. Census at that inspection was 17 residents in a 38-bed capacity.

No complaint investigations, enforcement actions, or fines are documented in the facility’s regulatory history. Over four years, the facility averaged 0.5 deficiencies annually, 90 percent better than Michigan’s 5.2 per year average.

Fountain View Retirement Village Grant serves seniors seeking assisted living with memory care in a long-established community, with recent inspections documenting safety equipment concerns requiring corrective action.

Overview of Harvest Pointe at Thornapple Manor

Harvest Pointe at Thornapple Manor is a 70-bed community at 2950 McKeown Road, Hastings, Barry County, MI, licensed as a Home for the Aged. Operated by the County of Barry and opened in 2023, the facility is very walkable with a walk score of 70, ranked 73rd of 702 Michigan communities, and situated at Hastings city center. The physical layout includes 50 residential units, with 20 configured for double occupancy.

The facility provides assisted living with rehabilitation services, short-term rehab, respite care, and memory care. Amenities include a full-service beauty salon, wellness program with on-site therapy and rehabilitation services, fitness center, free WiFi, personal linen service, concierge services, and ongoing life enrichment classes and opportunities. The facility does not accept Medicare or Medicaid and does not offer 24-hour staffing.

Initial licensing in February 2023 found two physical plant deficiencies: non-functioning exhaust ventilation in a common area toilet room and hot water temperature exceeding 120 degrees Fahrenheit in resident units. The facility addressed both promptly through corrective actions documented with submitted photos and videos.

The July 27, 2023 renewal inspection found zero deficiencies with substantial compliance. However, the October 9, 2024 renewal inspection documented one deficiency: medication administered to residents in applesauce or pudding without physician’s orders. At that inspection, census was 24 residents in the 70-bed capacity. Occupancy increased from 14 in July 2023 to 24 in October 2024.

No complaint investigations or enforcement actions are recorded.

Harvest Pointe at Thornapple Manor is configured for residents seeking assisted living with therapeutic rehabilitation and structured enrichment in a walkable, county-operated setting.

Overview of Cranberry Park of Milford

Cranberry Park of Milford is a 61-bed assisted living and memory care facility at 801 Whitlow Drive, Milford, Oakland County, MI, ranked 245th of 437 Michigan communities by bed count. Cranberry Park Milford LLC owns and operates the facility. The community is very walkable with a walk score of 70, ranked 73rd of 702 Michigan communities.

The facility provides 24-hour staffing with professional nursing staff, offering assisted living, memory care, short-term rehabilitation, and skilled nursing services, with respite care available. Amenities include private apartments and suites, beautiful courtyards with walking paths, housekeeping and linen service, a well-appointed dining room with restaurant-style meals, transportation, health and wellness monitoring, emergency call systems, physical occupational and speech therapy, and podiatry, dental, optometry, and psychological services. The facility does not accept Medicare or Medicaid.

From April 2018 through April 2024, Cranberry Park of Milford documented 4 deficiencies, 50 percent below Michigan’s average of 8. The facility averaged 0.5 deficiencies annually, 50 percent fewer than the state average of 1 per year.

A May 2023 renewal inspection found substantial compliance with zero deficiencies. However, a September 2022 complaint investigation identified 3 substantiated deficiencies: unresponsive care staff resulting in delays in care and protection violations, failure to ensure resident personal needs and protection consistent with service plan, and failure to notify family and physician of hospital transfer within required timeframe. A December 2023 complaint investigation found 1 deficiency related to failure to ensure ongoing competency evaluation of medication technicians after training, though an alleged medication error was not substantiated.

The most recent April 2024 renewal inspection found substantial compliance with zero deficiencies.

No fines or enforcement actions are documented.

Cranberry Park of Milford is a moderately sized assisted living and memory care community in a very walkable location, with recent improved compliance following earlier deficiencies in staff responsiveness and family communication.

Overview of Pomeroy Living Northville Assisted & Memory Care

Pomeroy Living Northville Assisted & Memory Care is a 109-bed community at 40033 West Eight Mile in Northville, Wayne County, Michigan, operating under Beacon Square Northville. The facility provides assisted living, memory care, outpatient rehabilitation, short-term rehabilitation and recovery, hospice and palliative care, and respite care. Administrator Sandra Salvati and Authorized Representative Manda Ayoub lead operations. Walk score is 83, very walkable and ranked 24th statewide.

The facility opened in 2016 with substantial compliance. The inspection record from 2016 was clean.

However, September 2023 identified four deficiencies with 21 residents on-site. Resident A’s tuberculosis screening chest x-ray was completed after move-in rather than prior to admission as required; resident B missed scheduled doses of Tramadol on August 18, August 30, and August 31 with no documented reason for non-administration; resident C did not receive prescribed Ketoconazole shampoo applications from September 2 through September 6 as ordered every other day. The menu posted in the dining room listed meals for a prior week instead of the current week.

The September 2024 complaint investigation reveals a more substantive gap. Resident A experienced four falls during his stay due to inadequate supervision. The facility used a tilt back wheelchair seat as a restraint method, which the investigation determined to be inappropriate. A resident in the facility’s care fell repeatedly while staff supervision was insufficient to prevent injury.

The pattern across both inspection periods centers on administrative and direct-care compliance. Medication administration documentation gaps, screening timing gaps, and prescribed care delivery gaps appeared in 2023. A substantiated supervision failure appeared in 2024. The facility has matched the Michigan average for deficiency rate over ten years, which means its performance is unremarkable in a statistical sense. Yet the specific content of those deficiencies involves core care functions. For residents requiring close oversight or those with fall risk, the supervision failure documented in September 2024 is material.

Families should ask about the progression from clean compliance, to recent deficiencies in medication administration and substantiated supervision failure leading to resident falls.

Frequently Asked Questions about Assisted Living in Michigan

What's the difference between assisted living and memory care in Michigan?

Assisted living in Michigan supports residents with daily activities (bathing, dressing, medication management) while preserving independence. Memory care is a specialized form of assisted living for residents living with Alzheimer's or dementia, and adds 24/7 secured environments, dementia-trained staff, and structured routines designed to reduce confusion and wandering.

What's the difference between assisted living and a nursing home in Michigan?

Assisted living in Michigan is a residential model focused on housing, hospitality, and help with daily activities. Nursing homes (skilled nursing facilities) provide 24/7 medical care from licensed nurses for residents with significant health needs, and are regulated more strictly under both state and federal CMS rules.

Does Michigan Medicaid cover assisted living?

Michigan Medicaid does not directly pay room-and-board for assisted living, but most states (including Michigan) offer Home and Community-Based Services (HCBS) waivers that can offset the cost of care services delivered inside a licensed community. Eligibility, waitlists, and covered services vary — check directly with the state Medicaid agency.

What is assisted living?

Assisted living combines private or semi-private apartments with help on activities of daily living (meals, bathing, dressing, medication management) and a social calendar — for residents who need some support but do not need 24/7 medical care.

How many assisted living facilities are listed on this page?

This page features 436 assisted living facilities in Michigan. Use the filters and comparison tools above to compare ratings, amenities, and pricing.

How do I choose the right assisted living facility in Michigan?

Start by matching the level of care offered to the resident's current and anticipated needs, then compare licensing status, staff-to-resident ratios, recent inspection results, and pricing. Tour at least two or three communities in Michigan, talk to current residents and families, and confirm what is included in the base rate versus billed as add-on services.

What should I look for when visiting assisted living facilities in Michigan?

Pay attention to staff interactions with residents, cleanliness and odor, food quality at meal times, the activity calendar, and how questions about pricing and care plans are answered. Ask to see the most recent state inspection report, the move-out / level-of-care-change policy, and a sample monthly bill that lists every fee.
