Administered by Jim Triana, Complete Care at Oak Ridge is a skilled nursing and rehabilitation facility in Charleston, West Virginia. Located on Association Drive, the 74-bed community focuses on short-term rehabilitation and post-acute care. Medicare, Medicaid, and private pay are accepted, giving families several options when arranging coverage for recovery stays following surgery, hospitalization, or illness.
The facility’s occupancy rate is 96% because there’s steady demand for rehabilitation services. Residents stay an average of about 119 days, which includes those in the early stages of recovery and transitioning into ongoing care. Rehabilitation services are a major focus. Respite care and 24-hour staffing support residents as they move from hospital care to home or longer-term nursing support.
Daily nurse staffing averages approximately 3 hours and 25 minutes per resident. Registered nurses provide 52 minutes of care per resident each day. Nurse aides contribute 2 hours and 4 minutes, while LPN/LVN staff provide 35 minutes. Those staffing levels support daily oversight and hands-on assistance during recovery.
As a newly opened facility, Complete Care at Oak Ridge completed its first annual state recertification survey with no deficiencies. Earlier findings were resolved before that review. The facility is in a car-dependent area of Charleston, so most visitors will need transportation. However, there’s parking and access for residents and visiting family members.
Families considering the facility should take a tour to see the care environment, room layouts, and daily routines.

Frequently Asked Questions about Nursing Homes in Charleston, WV
What's the difference between assisted living and a nursing home in West Virginia?
Assisted living in West Virginia is a residential model focused on housing, hospitality, and help with daily activities. Nursing homes (skilled nursing facilities) provide 24/7 medical care from licensed nurses for residents with significant health needs, and are regulated more strictly under both state and federal CMS rules.
Does West Virginia Medicaid cover nursing home care?
Yes — West Virginia Medicaid covers nursing home care for residents who meet income, asset, and medical-need eligibility requirements. Most CMS-certified nursing homes accept Medicaid as a primary payer once long-term-care eligibility is established.
What is nursing home care?
Nursing homes (also called skilled nursing facilities) provide 24/7 medical care from licensed nurses, rehabilitation services, and long-term custodial care for residents with significant health or functional needs.
How many nursing homes are listed on this page?
This page features 4 nursing homes in Charleston, WV. Use the filters and comparison tools above to compare ratings, amenities, and pricing.
How do I choose the right nursing home in Charleston, WV?
Start by matching the level of care offered to the resident's current and anticipated needs, then compare licensing status, staff-to-resident ratios, recent inspection results, and pricing. Tour at least two or three communities in Charleston, WV, talk to current residents and families, and confirm what is included in the base rate versus billed as add-on services.
What should I look for when visiting nursing homes in Charleston, WV?
Pay attention to staff interactions with residents, cleanliness and odor, food quality at meal times, the activity calendar, and how questions about pricing and care plans are answered. Ask to see the most recent state inspection report, the move-out / level-of-care-change policy, and a sample monthly bill that lists every fee.
