Town Village Tulsa operates as a 137-bed nursing home at 8222 S Yale Ave in the Silver Oaks neighborhood of Tulsa, Oklahoma. Founded as a voluntary nonprofit and owned by John Fonang, the facility provides nursing home care, independent living, and respite programming. It accepts Medicare, Medicaid, and private-pay residents.
The location sits 10.6 miles south of downtown Tulsa and 1.2 miles from Laureate Psychiatric Clinic and Hospital. Walk Score of 44 indicates somewhat walkable conditions with most errands requiring vehicles.
The facility’s CMS ratings present a complex picture.
Overall rating of 1 star reflects severe deficiencies. Health inspection rating of 1 star, 63.8% below Oklahoma’s state average, signals substantial regulatory violations. Staffing rating of 1 star, 22.9% below state average, points to critical nursing coverage gaps. Notably, quality measures achieve 4 stars, 41.4% above state average, which is a strength in a 1-star facility suggesting clinical outcomes exceed expectations despite infrastructure challenges.
Nursing staffing reaches 3 hours 52 minutes per resident daily, 16% below Oklahoma’s state average and ranking 36 of 46 state facilities. Registered nurse presence is severely constrained: four RNs work only 25 percent of operational days, leaving minimal medical oversight capacity. Licensed practical nurses total 26; certified nursing assistants, 67. The staff-to-resident ratio of 1.28:1 falls 21% below Oklahoma’s state benchmark.
Occupancy reaches 86.7%, exceeding Oklahoma’s 60.6% average by 43 percentage points and ranking 22 of 130 state facilities. This is a sign of strong local demand despite regulatory challenges. Medicaid comprises 77.5 percent of the current resident census; private pay, 19.8%; Medicare, 2.7%.
In March 2024, CMS imposed a $10,000 civil money penalty and suspended new Medicare/Medicaid admissions for 13 days due to regulatory violations.
Quality outcomes show unevenness: pressure ulcers in high-risk residents reach 8.5%, substantially exceeding Oklahoma’s 5.2% state average. Falls with major injury of 2.1% and incontinence rates substantially outperform peer benchmarks.
Town Village Tulsa serves predominantly Medicaid-funded long-term residents in Tulsa and holds 1-star regulatory ratings for health inspection and staffing, paired with 4-star quality measures performance.
