Worthington Christian Village is a 44-bed skilled nursing and rehabilitation facility at 165 Highbluffs Blvd, Columbus, Franklin County, Ohio 43235, operated by a church as a voluntary non-profit since 1985. Randall Richardson serves as Executive Director. The facility is owned by Lynn Borton.

This facility achieves exceptional staffing metrics. Registered nurse coverage totals 1 hour 40 minutes per resident per day, well above Ohio’s 38-minute average, with weekend RN hours reaching 1 hour 16 minutes (more than four times the state average of 25 minutes). Nurse aide hours average 4 hours 1 minute daily, 88% above state benchmark. Total nursing care runs 5 hours 41 minutes per day, ranking sixth statewide and 48% above Ohio’s 3-hour-50-minute average. Licensed practical nurse hours fall to 27 minutes daily, 53% below state average. Physical therapist hours are robust at 7 minutes per day. The facility carries a 5-star CMS rating across health inspection, staffing, and quality measures categories. Occupancy stands at 68.6%, lower than state average, suggesting available capacity. The census is predominantly short-stay Medicare residents (51%, averaging 27 days) and private-pay admissions (47%, typically 3–4 months). Medicaid comprises only 2% of new admissions.

Care quality and facility culture scores lag state averages slightly.

The January 2026 Ohio Department of Health complaint investigation identified a resident fall with actual harm: a resident with documented need for two-person assistance for bed mobility fell from bed while attended by a single nursing assistant, sustaining a leg fracture. The facility documented this as past non-compliance subsequently corrected. Prior inspections in September 2021 and March 2024 also revealed recurring deficiencies spanning medication management (psychotropic and antianxiety medications not reviewed for continuation, outside-parameter administration of blood pressure medications), food service (sanitation, portion control discrepancies, undated storage, hand hygiene lapses), infection control gaps (hand sanitization omitted during medical procedures), and antibiotic stewardship failures. All 11 citations across inspections are moderate or minor in severity; no critical or serious citations. The facility reports 5.5 citations per inspection, slightly above the state mean of 4.59.

Financial position: Revenue $4.7M, operating deficit $484,900 (2023). Payroll $3.4M represents 72.5% of revenue, ranking 13th statewide for payroll investment.
