Enjoy the warmth of Phoenix, AZ with Desert Peak Care Center, an astounding community that offers long-term care, acuity behavior care, dementia care, behavioral health counseling. Featuring specially designed accommodations in variations of private and semi-private rooms, residents have plenty of choices that match their preferences for the utmost comfort and safety. A compassionate and highly trained team attends to residents’ daily living activities and personal needs.
Have fun and live actively with a full calendar of dedicated activities tailored to your leisure and wellness. Scheduled transportation, visiting physicians, and a comfortable dayroom are also available to meet residents’ healthcare needs. Indulge in healthy and delicious meals catering to dietary needs and preferences. The community is great for those who need supplementary care to live the finest retirement experience.
Sagewood sits in Phoenix’s Desert Ridge West neighborhood, a car-dependent pocket of the city (Walk Score 12) where the nearest errands are a drive away. The three-story building has been here for 20 years, operated by LCS Desert Ridge LLC under the LCS brand, and it covers a lot of ground: independent living, assisted living, memory care, and nursing home care all under one roof at 4555 E Mayo Blvd. Executive Director Mr. Stewart Ingram oversees the campus.
What Sagewood is selling, functionally, is the ability to age in place without switching facilities. Care levels are available across the full spectrum, so a resident who moves in for independent living can shift to skilled nursing without packing up and starting over somewhere else. Short-term rehab and respite stays are also on offer, which makes the campus useful for families navigating post-acute recovery or caregiver breaks. Memory care is available with a secured perimeter in place.
The programming side runs through LCS Signature Programs, including the LifeCare plan and the HealthyLife wellness track, the latter built around fitness, nutrition, and general well-being. In terms of physical amenities, the campus includes a fitness center, a spa, walking paths, a wellness center, and concierge services, with religious services also available. Apartment options span one- and two-bedroom layouts, with square footage ranging from roughly 763 to over 2,300 square feet.
Sagewood is not currently listed as a CMS-certified Medicare or Medicaid provider. Families planning around federal coverage pathways will need to look elsewhere or explore Arizona’s state-level assistance programs independently.
For older adults who want a tiered campus where care can follow them across multiple stages, with branded programming and substantial apartment options, Sagewood covers that range. The non-certification status and car-dependent location are the two variables most families will want to factor in early.
Fellowship Square of Phoenix sits in Phoenix’s North Mountain Village neighborhood at 2002 W Sunnyside Ave, 46 years into operation and affiliated with Christian Care. The campus covers four levels: nursing home, assisted living, independent living, and memory care. HonorHealth John C. Lincoln Medical Center is 2.34 miles away. The Walk Score is 58, which is moderately walkable; some errands are reachable on foot, but a car handles most of the neighborhood’s geography.
Rehabilitation services are available on-site. The facility’s own overview also references occupational, speech, and physical therapy, along with post-surgical and post-hospital care. Memory care is on campus for residents living with Alzheimer’s or dementia.
On the daily life side, the campus describes its dining as fresh, well-prepared food with multiple options. Activities listed in the facility’s overview include arts and crafts, film viewings, and religious programming.
Fellowship Square is not CMS-certified, which means Medicare and Medicaid are not in play here. Families will need to plan around private pay or explore Arizona’s state-level assistance programs.
For older adults in the north Phoenix area who want a faith-affiliated campus where care can shift across four levels without a change of address, from independent living through skilled nursing and memory care, Fellowship Square has held that position for over four decades.
Stoneridge Hospice delivers home-based palliative and end-of-life care across the Phoenix, Arizona region. Administered by Patti Brazel under CEO Rollie Seebert, the agency operates its central coordination office in the Desert Ridge West neighborhood. The clinical framework relies on an RN Model of Care, which assigns a specific registered nurse case manager to each patient to manage symptoms and coordinate routine care directly inside the home.
The surrounding neighborhood has a walkability score of 58 out of 100, meaning staff and visiting relatives can manage some errands on foot, and HonorHealth Scottsdale Thompson Peak Medical Center sits about three miles away. The care model combines physical pain management with social work services, counseling, spiritual care, and bereavement support for family members.
Holding an ACHC accreditation, the provider brings six years of operating experience to its services and accepts Medicare, Medicaid, VA benefits, and private insurance.
Families can schedule an initial consultation to confirm insurance coverage and determine hospice eligibility.

Frequently Asked Questions about Nursing Homes in Phoenix, AZ
What's the difference between assisted living and a nursing home in Arizona?
Assisted living in Arizona is a residential model focused on housing, hospitality, and help with daily activities. Nursing homes (skilled nursing facilities) provide 24/7 medical care from licensed nurses for residents with significant health needs, and are regulated more strictly under both state and federal CMS rules.
Does Arizona Medicaid cover nursing home care?
Yes — Arizona Medicaid covers nursing home care for residents who meet income, asset, and medical-need eligibility requirements. Most CMS-certified nursing homes accept Medicaid as a primary payer once long-term-care eligibility is established.
What is nursing home care?
Nursing homes (also called skilled nursing facilities) provide 24/7 medical care from licensed nurses, rehabilitation services, and long-term custodial care for residents with significant health or functional needs.
How many nursing homes are listed on this page?
This page features 14 nursing homes in Phoenix, AZ. Use the filters and comparison tools above to compare ratings, amenities, and pricing.
How do I choose the right nursing home in Phoenix, AZ?
Start by matching the level of care offered to the resident's current and anticipated needs, then compare licensing status, staff-to-resident ratios, recent inspection results, and pricing. Tour at least two or three communities in Phoenix, AZ, talk to current residents and families, and confirm what is included in the base rate versus billed as add-on services.
What should I look for when visiting nursing homes in Phoenix, AZ?
Pay attention to staff interactions with residents, cleanliness and odor, food quality at meal times, the activity calendar, and how questions about pricing and care plans are answered. Ask to see the most recent state inspection report, the move-out / level-of-care-change policy, and a sample monthly bill that lists every fee.
