The nonprofit Wesley Gardens Corporation runs the facility, which accommodates an even mix of insurance arrangements; Medicare, Medicaid, and private-pay residents each represent roughly one-third of new admissions. Most stays average about 143 days, positioning the facility for post-acute rehabilitation transitions. Active resident governance through ongoing resident council meetings is standard practice.
Direct nursing staffing delivers 4 hours 38 minutes per resident per day, the highest ratio among facilities in this sample, and 33% above New York’s average. This staffing measure reflects meaningful commitment: 12 RNs, 39 LPNs, 69 CNAs, plus 7 clinical nurse specialists in supervisory roles. All-staff shifts average 8.2 hours, slightly above the state norm.
Across 21 inspections since 2015, the facility accumulated 55 citations, more than four times the state average. The June 2024 survey, the most comprehensive finding, identified 22 deficiencies across care delivery (ADL assistance, pain management, medication management), administrative compliance (care planning, nursing staffing documentation), and physical plant (construction standards). The August 2024 follow-up showed correction of most items. The August 2025 inspection recorded just one isolated violation regarding notification of resident condition changes; a marked tightening suggesting operational discipline.
