An earlier January 2024 complaint survey identified six deficiencies: absent resident participation in care plan development, non-functional call bell in a resident’s bathroom, inaccurate baseline assessments, staffing information not posted, dietary preferences ignored, and missing hospice documentation. The inspection summary explicitly states the facility shows “a pattern of issues” and “deficiencies have persisted over time with similar themes, indicating ongoing challenges” in care planning, assessments, dietary compliance, and maintenance.
