- BROOKDALE CREEKSIDE underwent a comprehensive inspection on August 13, 2024, resulting in 16 violations of state standards. Upon reviewing the inspection findings, it is evident that the majority of these deficiencies fall under the Life Safety Code category, with only one Health Code violation listed. The Life Safety Code issues include fire safety and building maintenance concerns, such as inadequate smoke detector sensitivity checks, insufficient exit signs, and non-compliant electrical systems. Additionally, there were several instances of incomplete or missing documentation for fire alarm and sprinkler system inspections and maintenance. The single Health Code deficiency pertained to the facility's smoking policy. It is worth noting that correction dates range from May 22, 2022, to July 28, 2022. No enforcement actions were found in relation to these inspection findings.
