Three recurring violations involving the failure to report to the national health safety database remained unaddressed across 2022 and 2023 surveys. A June 2024 complaint investigation substantiated allegations of abuse and neglect, both corrected within weeks. No fines, immediate jeopardy citations, or license restrictions appear in the public record.
