- April 2, 2025 initial licensing survey found 21 deficiencies including failure to provide sufficient housing, incomplete emergency and tuberculosis programs, and lack of required staff training and supervision.
- Deficiencies included failure to comply with Minnesota Food Code, incomplete resident records and contracts, and inadequate medication management assessments and plans.
- The facility was denied an assisted living license due to widespread noncompliance with Minnesota Statutes Chapter 144G and missing required grievance and electronic monitoring postings.
