Earlier inspections identified issues with resident care, including failure to follow care plans that resulted in a fall with serious injury, medication transcription errors, and inadequate hygiene and infection control practices. Complaint investigations substantiated abuse involving a resident who was forcibly transferred, causing injury and immediate jeopardy that was later resolved, as well as failures in timely family notification and incident reporting.
