The facility maintains 24-hour staffing with on-site licensed nurses and certified nursing assistants; nursing hours per resident per day exceed the Ohio state average by 14 percent. Located 2.9 miles from Middletown’s city center, the facility does not accept Medicare or Medicaid.

The inspection record over six years reveals substantive and recurring compliance failures across multiple care domains. Thirteen inspections (three comprehensive health surveys conducted in July 2019, March 2023, and June 2025; ten complaint investigations between March 2023 and February 2025) documented 59 total deficiencies. Ten subsequent complaint investigations identified persistent patterns: fall prevention failures with missing perimeter mattresses and inaccessible call lights (August 2023, 2 deficiencies); proprietary medication sourcing violations affecting 41 residents spanning three inspections (November 2023, 4 deficiencies); delayed physician notification of abnormal laboratory results (February 2024 and recurring thereafter); inadequate incontinence and catheter care delivered without timely assistance (February 2024, 4 deficiencies); violations of preadmission mental health screening requirements and absence of documented quarterly care conferences affecting five residents (March 2024, 8 deficiencies); a preventable fall with actual harm—resident sustained femur and humerus fractures requiring open reduction internal fixation—resulting from failure to provide adequate supervision during bed mobility (March 2024); substantiated physical abuse by nursing staff involving forcible restriction of resident movement and verbal abuse; failure to coordinate continuity of care with outpatient dialysis, resulting in a resident with end-stage renal disease going four days without hemodialysis and suffering cardiopulmonary arrest and death (marked Immediate Jeopardy in April 2024, 3 deficiencies); significant unintended weight loss in residents without physician or family notification (May 2024, 3 deficiencies); inadequate colostomy care with recurrent skin breakdown and infrequent pouch changes (February 2025, 1 deficiency); failure to notify resident representatives of changes in health status (January 2025, 4 deficiencies); smoking area accessibility deficiencies (January 2025, 1 deficiency); and structural defects including damaged ceiling tiles in resident rooms, unmet personal hygiene needs, food storage contamination including insect-infested refrigeration, cold food service, and absence of an implemented Legionella prevention program (June 2025, 6 deficiencies).

Amenities include prepared meals, life enrichment activities, on-site licensed nurse availability, residential options, assistance with activities of daily living, housekeeping, laundry and household maintenance, and outdoor patio and garden areas.
