A March 11, 2025, relicensure survey found no deficiencies. A November 5, 2025, complaint investigation substantiated one serious deficiency: the facility failed to provide adequate nursing supervision to ensure safe transfer needs were met for one resident, resulting in a fall and hospitalization.

The facility’s policy required two staff members for all transfers, but one certified nursing assistant transferred the resident alone, violating protocol and directly contributing to a fall and subsequent resident death. The staff member was terminated. A revisit on January 12, 2026, confirmed all deficiencies were corrected by December 31, 2025. Administrator Michelle Netters.
