On 06/02/2022, an LPA noted that four COVID‑positive residents and staff cases had not been reported within the required 24‑hour window, violating Section 1, 2, 3, 4, 5, 6, 7 of the reporting requirements. A second visit on 06/22/2022 identified multiple deficiencies (Types A and B) related to medication administration protocols, eviction notice requirements, and missing written admission agreements. Each deficiency carried its own due date (e.g., 06/23/2022, 06/29/2022) and the facility was warned that failure to correct could result in civil penalties.

Overall, Sonnet Hill demonstrated strong physical and safety compliance during its initial inspection and the routine 1‑year check. However, subsequent case‑management investigations uncovered recurring gaps in reporting, documentation, and procedural compliance. Addressing these deficiencies promptly is essential to maintain licensure status and protect resident safety.
