SUNDALE SENIOR LIVING - THE LEXINGTON CENTER underwent a comprehensive inspection on January 23, 2024, resulting in 7 state standard violations. The Health Code deficiencies included failure to provide required counseling to residents who self-administer medications or maintain a written counseling record, which was corrected by January 31, 2024. Life Safety Code deficiencies were more prevalent, with issues related to fire safety and building maintenance. These included inadequate exit signs with emergency power, lack of a written contract for fire alarm system inspections, non-compliance with sprinkler system inspection requirements, poor building repair, substandard floor, ceiling, and wall finishes, and an unsanitary dishwashing area.
