Inspection Report Summary for The Courte at Citrus Heights
The inspection of the 48‑bed group home “Courte at Citrus Heights” was conducted in July 2021 following a complaint lodged in February. The licensing evaluator, Bethany Mirlohi, arrived unannounced and performed COVID‑19 screening before meeting with the administrator and staff. The evaluator interviewed seven caregivers and reviewed resident charts, care plans, and physician reports for each allegation.
The evaluator examined nine separate allegations. They ranged from claims that the facility failed to provide basic bathing and incontinence care for a resident with dementia, to accusations that the facility did not report a scabies outbreak or seek timely medical attention, and to concerns that residents sustained unexplained injuries and were not properly supervised or spoken to inappropriately. In each case the evaluator gathered evidence that the resident’s behaviors—aggression, refusal of care, wandering, and falls—contributed to the incidents. Documentation showed that staff attempted to provide shower or bath services when the resident would accept, that they tried to redirect wandering residents, and that medical treatment for scabies was eventually obtained and recorded. The evaluator noted that no formal scabies diagnosis had been documented in January 2021, and that the scabies outbreak was reported only in March when the medical director confirmed cases and prescribed medication. For the injury allegation, chart reviews indicated bruises on the resident had appeared on multiple dates but the evaluator could not conclusively link them to supervisory failure.
Throughout the investigation the evaluator found the allegations to be either “unsubstantiated” or “unfounded.” The patterns that emerged highlighted the challenges of caring for residents with dementia who exhibit aggression, refusal of care, and wandering, rather than deficiencies in staff training or policy. The evaluator emphasized that care plans were updated annually and that staff were following procedures for redirection and for notifying medical providers. No violations of licensing requirements were identified, and the facility was instructed to continue monitoring resident behavior, maintaining accurate documentation, and promptly reporting infectious outbreaks as required.
