The facility was cited for two Health Code deficiencies related to infection control policies and procedures, which were corrected by January 15, 2022. Additionally, four Life Safety Code deficiencies were identified during the November 16, 2020, inspection, including inadequate lighting levels, missing insect screens on operable windows, and insufficient staff training on fire extinguisher use.
