The facility failed to ensure locked gates complied with licensing standards (corrected by May 28, 2019), conduct and document fire drills (corrected by July 13, 2019), provide exit signs with emergency power (corrected by May 23, 2019), maintain adequate lighting levels (corrected by May 23, 2019), test smoke detectors for sensitivity (corrected by May 29, 2019), and conduct an annual fire marshal inspection (corrected by May 29, 2019). No Health Code deficiencies were listed. The facility had previously corrected a single Health Code violation related to service planning on August 10, 2021.
