The most recent inspection on June 19, 2025, found the facility in compliance with Medicare/Medicaid participation requirements and Life Safety Code standards, with no deficiencies noted. Prior inspections showed a pattern of Life Safety Code and emergency preparedness deficiencies, including issues with outdoor smoking area maintenance, emergency generator testing, and fire safety equipment, as well as care-related deficiencies such as discharge planning, medication management, and infection control.
