The A & A Care and Wellness Facility, a 740 group home located at 16055 Sequoia Street in Hesperia, California, was subject to two unannounced inspections by the California Department of Social Services Community Care Licensing Division in May and October 2023. Both visits were prompted by the same public complaint (control number 56‑AS‑20230427101245) and investigated a range of alleged deficiencies involving staffing, background clearance, meal quality, medication management, resident supervision, language barriers, and dietary compliance.

During the May inspection, nine allegations were examined. These covered concerns that the home was understaffed, that employees lacked required background checks or qualifications, that meals were nutritionally inadequate or insufficient in quantity, and that medication storage and logging practices were insecure. The evaluator conducted a facility tour, reviewed staffing records, training logs, food inventories, and medication logs. The findings were uniformly **Unsubstantiated**; documentation confirmed that staffing levels met regulatory requirements, all employees had valid background clearances and training, the kitchen stocked adequate fresh and non‑perishable food, and medications were stored in a locked cabinet and accurately recorded. No evidence or witness testimony supported any of the claims, and no enforcement actions were recommended.

The October inspection focused on six new allegations: resident over‑medication, leaving residents alone, requiring a resident to hide from visitors, improper medication logging, language‑barrier communication failures, and deviation from a resident’s special diet. An interview with the caregiver, staff, and residents, coupled with a medication audit that found no discrepancies, again yielded **Unsubstantiated** findings for every allegation. The evaluator confirmed that medication administration records were maintained correctly, staff had appropriate language support, and residents were not left unattended.

Across both investigations, the same evaluator documented consistent patterns: complaints surfaced, but the home’s administrative records and on‑site practices consistently met state standards, and no evidence was found to substantiate the alleged violations. While the repeated complaints suggest continued vigilance by oversight staff, the inspections indicate that the facility has maintained compliance with California’s group‑home licensing requirements as of the dates examined.
