A 2015 biennial construction survey cited five deficiencies related to fire-resistance ratings, hand grips, building equipment, exit signage, and ventilation. An August 2, 2017, annual survey cited three deficiencies: failure to serve required milk, failure to administer medications as ordered, and failure to ensure Special Care Unit staffing.

An August 17, 2017, construction survey cited fourteen deficiencies spanning fire detection and sprinkler protection, exit signage, mold and odors, electrical safety, emergency lighting, fire alarm functionality, and hot water temperature. A September 26, 2019, construction survey cited fourteen deficiencies in unlabeled emergency release switches, corridor obstructions, plumbing hazards, unsafe portable oxygen, inadequate fire safety rehearsals, kitchen hood fire suppression system maintenance, electrical safety, ceiling penetrations, sprinkler system maintenance, door hardware, and ventilation. A June 24, 2020, complaint investigation and COVID-19 infection control survey substantiated deficiencies in staff and resident screening, PPE use, infection control, and staffing levels resulting in 51 resident infections, 13 staff infections, and 12 resident deaths. A December 10, 2020, follow-up survey cited three deficiencies: failure to ensure CPR-trained staff for 24 of 42 shifts, failure to ensure referral and follow-up for residents, and failure to implement fingerstick blood sugar orders.

A March 29, 2021, follow-up and annual survey cited four deficiencies: failure to refer to mental health provider for psychiatric medication management, failure to implement weight monitoring orders, failure to administer medications as ordered for six residents, and inaccurate medication administration records. A March 4, 2022, follow-up and complaint investigation cited six deficiencies: failure to ensure staff had no substantiated findings on the Health Care Personnel Registry, failure to complete criminal background checks for two staff, failure to supervise one resident resulting in 10 falls in 3 months, failure to contact providers timely, medication administration errors, and inaccurate controlled substance records. A June 1, 2022, follow-up survey cited five deficiencies: medication administration errors, failure to ensure self-administration physician orders, failure to store expired medications separately, inaccurate controlled substance records, and failure to enforce COVID-19 mask policies. A February 3, 2023, annual and follow-up survey cited six deficiencies: failure to ensure medication aide exam passage, failure to serve therapeutic diets as ordered, failure to respect resident blood sugar monitoring rights, medication administration errors, failure to maintain medication security, and failure to implement proper glucometer infection control.

A May 10, 2024, annual survey and complaint investigation cited fifteen deficiencies: failure to restrict Special Care Unit window openings, lack of handwashing sinks adjacent to medication storage, malfunctioning climate control, failure to screen for tuberculosis on admission, failure to notify providers of refused insulin doses, failure to implement physician orders, inadequate table service and therapeutic diets, insufficient meal spacing, failure to serve required milk and water, lack of shared bathroom privacy, medication administration errors, inaccurate controlled substance records, and failure to notify guardian within 24 hours. An August 14, 2024, follow-up survey cited five deficiencies: failure to restrict or alarm window openings, failure to secure hazardous cleaning agents, failure to notify providers of elevated blood sugar, failure to ensure self-administration physician orders, and inaccurate controlled substance records. An August 28, 2025, annual inspection cited six deficiencies: failure to restrict window openings, failure to ensure medication aide training and testing, failure to notify provider of compression sock non-compliance, medication administration errors including crushing delayed-release medication, failure to ensure infection control during medication passes, and failure to ensure self-administration physician orders. Occupancy has grown from 19 residents in 2017 to 61 in 2024.

The June 2020 COVID-19 infection control failures resulting in 51 resident infections and 12 deaths represent a critical patient safety incident. Recurring deficiencies spanning nine years in medication administration, staff training, facility maintenance, fire safety, window security, and infection control indicate systemic failures unresolved despite multiple corrective action plans. The May 2024 fifteen-deficiency and August 2025 six-deficiency inspections confirm persistence of core failures.

Before placement, families should ask about the June 2020 COVID-19 incident, and systemic safeguards for infection control implemented since, including proper medication pass techniques.
