Substantive deficiency themes included failure to monitor changes in resident condition, incomplete RN assessments, medication errors, inadequate restraint assessments, insufficient staff training, and fire and life safety instruction gaps. Kitchen cleanliness, infection prevention, and licensing rule compliance were also cited. Some deficiencies were corrected on revisits; others persisted initially. No fines, immediate jeopardy findings, enforcement actions, or substantiated complaints were reported. The facility has not undergone inspection since March 2023, leaving a three-year gap in regulatory oversight.
