The regulatory record documents severe and persistent care quality and operational failures spanning medication management, staffing, abuse prevention, and compliance systems. Over two years, Cascade Terrace logged 61 deficiencies; 30.5 annually, 355 percent above Oregon’s 6.7-deficiency benchmark.
