The inspection record shows 17 total citations since 2023, below the Wisconsin average of 27.3, at 3.4 per inspection against a state average of 4.19. One critical citation is on record, corrected, with Abuse, Neglect, and Exploitation as the leading deficiency category at 4 deficiencies. A $10,000 civil money penalty was assessed in May 2023, 89 percent below the Wisconsin average fine amount. All CNAs and RNs are direct employees, with contractor hours at just 4.1 percent of the total.
