A Walk Score of 94 puts the neighborhood in the “Walker’s Paradise” range, meaning groceries, errands, and day-to-day stops are mostly doable on foot, with downtown Milwaukee less than a mile away.

Chiara Housing and Services Inc is the legal operator, Glenwood Choban administers the community, and Samantha Jemison holds a leadership role on-site. This is a newer property, still building its track record, but what it offers right now is a full spectrum of care, real dining, and a location most people would call genuinely walkable.

With rehabilitation and respite services, the community also appeals to those seeking short-term care. Promoting an active lifestyle, the community has a 14-hour activity window that encompasses arts and crafts, puzzles, games, and exercise. The community’s amenities stand out with its focused, care-forward environment, which appeals to families seeking a balance of lifestyle and care. However, the community does not accept Medicare, making private pay or Medicaid the only feasible funding option. 

With its dedicated memory care program in an intimate, relationship-centered setting and assisted living support, Prairie Home is an ideal choice for those who require specialized dementia care but prefer a community-rooted atmosphere in the Holmen and greater La Crosse area.
