The facility offers independent living, assisted living, memory care, and respite care for residents including those with traumatic brain injury, advanced age, irreversible dementia and Alzheimer’s disease, and HCBS-compliant needs. Licensed as a Class CNA (non-ambulatory) facility since October 1991, Villa St. Francis has operated for 37 years. Monthly rates range from $4,910 to $10,140. Housing includes studio, one-bedroom, and two-bedroom units in a three-story building; pets are allowed. On-site amenities: convenience store, banking center, beauty salon, massage therapist, and pastoral care for residents of all faiths. Services: 24-hour staffing, meals, housekeeping, laundry, transportation, and fitness and recreation programs. In October 2025, the Wisconsin Department of Health Services conducted a standard survey and found four substantive deficiencies. One involved involuntary discharge procedures: a resident admitted July 30, 2025, fell during a transfer the next day, sustaining a fracture. On August 21, the facility issued a 30-day discharge notice claiming care was beyond its capability, despite the hospital denying the resident needed rehabilitation. The resident was returned to the facility on a Hoyer lift, and subsequent physical therapy recommended a higher level of care. Wisconsin regulation allows involuntary discharge only for specific reasons. The facility’s stated reason did not align with these allowable grounds. A second deficiency involved medication administration. A third deficiency addressed health monitoring: a resident with Type 2 diabetes, ordered to have blood sugar checked three times daily with notification to the physician if readings exceeded 350, had seven instances above 350 in September. Only one reading was documented as reported to the physician, after the resident’s power of attorney called staff. The remaining six instances showed no record of physician notification. A fourth deficiency involved fire safety: the facility did not schedule an annual fire inspection for 2024; the previous fire inspection was April 14, 2023.
On December 19, 2025, the Wisconsin Department of Health Services issued a Notice of Violation and enforcement order imposing a $550 forfeiture ($350 for the health monitoring failure and $200 for the medication error), with a reduced option of $357.50 if the facility accepted the violation without appeal. The department ordered corrective measures within 45 days, including revised involuntary discharge procedures with specific written notice requirements and resident rights information. On February 24, 2026, a verification visit confirmed all four deficiencies were corrected and no new deficiencies were identified.
